 Janet Andrews, being first duly sworn, states as
 follows:
 1. My name is Janet Andrews_ I am thirty-two (32)
 years of age and live in Coeur d'Alene, Idaho, with my husbands
 Jacks and our two sons_ Joshua and Levi.
 2. i was married to Jack Andrews in June of 1976.
 Our son Joshua was born on January 18, 1977_ and Levi was born
 February 9_ 1980®
 3. One Sunday evening in November, 1980_ Joshua
 (_Josh _) complained he had a stomachache. His belly felt hard
 to the touch and seemed fullo I thought he was constipated and
 decided to take him along the next day to an already scheduled
 doctor's appointment for Levi, who needed a throat culture.
