 1. My name is John C. Mer_itto I am forty-two (42)
 years of age and live in Durham, North Carolinaź
 2o ! received my undergraduate education from
 Rampton Institute in Hampton_ Virginia° I studied medicine at
 Howard University College of Medicine. _ completed my Intern-
 ship at the University of Illinois Hospitalf Chicago_ lllinois_
 and performed my residency in Ophthalmology at the University
 of Illinois Eye and Ear Infirmary. in Chicago_ Illinois°
 3. Following completion of my Ophthalmic Residency_
 I received a one-year Fellowship in Pediatric Ophthalmology to
 continue my post_graduate studies at the Bascom Palmer Eye
 Institute in the School of Medicine at the University of Miami
 in Florida.
