 John Bickers, being first duly sworn, states as
 follows:
 I. My name is John Bickers. I am fifty-six (56)
 years of age and I live in Metairie, Louisiana.
 2o I received my B.A. degree from the University of
 Georgia in 1950, and I received my medical education at the
 Medical College of Georgia located in Augusta, from which I
 graduated in 1954. I conducted my Internship at St. Louis City
 ....... Hospital and received my Residency and specialty training at
 Tulane University School of Medicine from 1957 through 1961.
 3. I am a licensed physician in the state of
 Louisiana and have received board certification in Internal
 Medicine and in Medical Oncology.
