 Ronald Stephens, being first duly sworn, states as
 follows:
 I. My name is Ronald Stephens. I am forty_seven
 (47) years of age and I live°in Overland! Park, Kansas.
 2. ! received my Bachelor of Arts from Wichita
 State University in 1961 and my basic medical school training
 from the Kansas University School of Medicine from which I
 graduated in !965. My internship was conducted at Brian
 Memorial Hospital in Lincoln, Nebraska_ between 1965 and
 1966. Thereafter_ I joined the Public Health Service (PHS),
 where X worked in Prince Georges County, Maryland, on heart
 disease control and with the University of Maryland Physical
 Rehabilitation Program. My medical residency was conducted at
 the University of Kansas Medical School from 1968 to 1970 in
